Group Request Form

Use this form for requests from community groups or businesses to tour the facility or to volunteer for a group activity. Check the activities below and if they will want to use the Community Room for their group. Check YES if you want to be the point person for the event (tour and/or activity) and include any additional details. If they ask to book the Community Room, Admin staff will follow up with them to complete a Community Room request and will put it on the schedule. Program staff will respond to the Requestor listed for the group if they have asked to volunteer, to let them know what volunteer activity is available. Admin staff will schedule any tours with the appropriate staff member.
